2) Set surface casing per Rule 317d, cement to surface. Setting surface casing less than the approved depth is a permit violation unless prior written approval is obtained from the COGCC.
3) If completed, provide cement coverage from TD to a minimum of 200' above D Sand. Verify coverage with cement bond log.
4) If dry hole, set 60 sks cement from 50’ below D Sand base to 100’ above D Sand top, 40 sks cement 50’ above Niobrara top, tag 50 sks cement ½ out, ½ in surface casing, 10 sks cement top of surface casing, cut 4’ below GL, weld on plate, 5 sks cement in rat hole & 5 sks cement in mouse hole.

03/21/2019 | Plugging:
1) Provide 48 hour notice of MIRU via electronic Form 42.
2) Shoe plug (1550'-1350'): Tag plug 50’ above surface casing shoe.
3) Surface plug: Cement from 50' to surface in casing and annulus.
Bradenhead:
1) If there has not been a reported Bradenhead test within 60 days of plugging this well, prior to starting plugging operations a Bradenhead test shall be performed. The Form 17 shall be submitted within 10 days of the test.
2) If before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
3) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
Venting during plugging:
Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
Flowlines:
Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.
