1) Provide 24 hour notice of MIRU to Bo Brown at 970-397-4124 or e-mail at bo.brown@state.co.us. 2) Comply with Rule 317.i and provide cement coverage from end of production casing to a minimum of 200' above Niobrara. Verify coverage with cement bond log. 3) Comply with Rule 321. Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

Post Plugging Reporting • Form 17 Bradenhead Test Report shall be submitted within 10 days of the test. • File electronic Form 42 once flowline abandonment is complete.

Plugging • Provide 48 hour notice of plugging MIRU via electronic Form 42. • Surface casing shoe already isolated with primary cement. Production casing stub plug will be inside of surface casing and if plug is not circulated to the surface then the plug shall be tagged. Max depth to top of stub plug is 50’ above the depth of the casing cut, 530' or shallower for the planned cut at 580’. • After pumping stub plug wait on cement per design and monitor for fluid migration. Contact COGCC Engineer for revised plugging plans if well builds pressure when shut in, prior to pumping next plug. • Run CBL on surface casing from 580-0'. Contact COGCC Engineer for revised plugging plans if cement bond log shows any area of bond that is less than expected, a very good bond throughout.. • Place a 25 sack plug at the surface, all other plugs shall have at least 100' of cement left in the casing. • Properly abandon flowlines as per Rule 1103.

Bradenhead Testing Prior to starting plugging operations, a bradenhead test shall be performed. If the initial shut-in bradenhead pressure is greater than 25 psi, notify COGCC Engineer for sampling requirements prior to beginning the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
