Note surface casing setting depth change from 730’ to 805’. Increase cement coverage accordingly and cement to surface. 1) Provide 24 hr notice of spud to Bo Brown at 970-330-6085 or email at bo.brown@state.co.us. 2) Set surface casing per Rule 317d, cement to surface. Setting surface casing less than the approved depth is a permit violation unless prior written approval is obtained from the COGCC. 3) If completed, provide cement coverage from the first casing shoe to a minimum of 200' above Niobrara. Verify cement with a cement bond log. 4) Comply with Rule 321. Run and submit Directional Survey from TD to base of surface casing. Ensure that the wellbore complies with setback requirements in commission orders or rules prior to producing the well.

1) Flaring of natural gas shall comply with COGCC Rule 912. 2) Submit requests to flare annually to COGCC on Form 4 (Sundry Notice), which discusses timing to connect to sales line, plans to develop infrastructure, and justification for continued venting/flaring. 3) Provide current (< 1 year old) gas analysis including H2S with each Sundry Notice (within 30 days of commencing flaring if not yet reported). 4) Report flaring volumes on Form 7, Operator’s Monthly Report of Operations. 5) The operator is required to obtain and maintain any required air permits from CDPHE. 6) The expectation for permanent flares controlling separators will be an enclosed combustion device with a 98% design destruction efficiency for hydrocarbons. 7) Notify the local emergency dispatch or the local governmental designee of natural gas flaring. 8) This approval is good for one year. New application must be made prior to 1 year elapsed from start date or sooner if the conditions change. 9) Presence of hydrogen sulfide > 1ppm should be reported on a Form 42, Field Operations Notice. 10) Gas analysis including H2S should be submitted via Sundry Notice and EDD once available.
