COGCC ENG COA - SME 1) Note surface casing setting depth change from 400’ to 650’. Increase cement coverage accordingly and cement to surface. Set surface casing per Rule 317d, setting surface casing less than the approved depth is a permit violation unless prior written approval is obtained from the COGCC. 2) Provide 24 hr notice of spud to Colby Horton at 970-467-2517 or e-mail at colby.horton @state.co.us . 3) If completed, cement coverage shall be at a minimum from the TD to 200' above Niobrara and at minimum set stage cement from 1250’ to 50’ above the surface casing shoe. Verify all placed cement with cement bond log and cementing tickets. 4) If dry hole, set 60 sks cement from 50’ below D Sand base to 100’ above D Sand top, 40 sks cement 50’ above Niobrara top, 40 sks cement 1350’, 50 sks cement ½ out, ½ in surface casing, 10 sks cement top of surface casing, cut 4’ below GL, weld on plate, 5 sks cement in rat hole & 5 sks cement in mouse hole. Restore surface location.

Properly abandon flowlines per Rule 1105. If flowlines will be abandoned in place, include with the Form 27: pressure test results conducted in the prior 12 months as well as identification of any document numbers for a COGCC Spill/Release Report, Form 19, associated with the abandoned line.

Consistent with Rule 911.a, a Form 27 must be approved prior to cut and cap, conducting flowline abandonment, or removing production equipment. Allow 30 days for Director review of the Form 27; include the Form 27 document number on the Form 44 for offsite flowline abandonment (if applicable) and on the Form 6 Subsequent.

Plugging 1) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations and electronic Form 42 Notice of Plugging Operations 48 hours prior to mobilizing for plugging operations.   2) Plugs and squeezes will be placed as stated in the Plugging Procedure section of the approved NOIA unless revised by COA or prior approval from COGCC is obtained. 3) The wellbore must be static prior to placing cement plugs which are to be a minimum of 100' in length for all but surface plugs. Mechanical isolation requires a 25' cement plug, minimum. For plugs not specified to be tagged, a tag is required if circulation is not maintained while pumping plug and displacing to depth. Tag at tops specified or shallower. Notify COGCC Area Engineer before adding cement to previous plug. 4) Place a 50' plug (minimum) at the surface, both inside the inner most casing and in all annular spaces. Surface plugs shall be circulated to surface. Confirm cement to surface in all strings during cut and cap. 5) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed. 6) Operator must wait a sufficient time on all plugs to achieve the intended design. If at any time during the plugging there is evidence of previously unreported pressure or fluid migration, contact COGCC Area Engineer before continuing operations. 7) Plugging procedure has been modified as follows, Plug #1 - 7675', CIBP with 2 sx of cement. Plug #2 - 6800', CIBP with 2 sx of cement. Plug #3 - 2200-2000', 25 sx cement casing plug. Plug #4 - 1200-1000', 30 sx cement casing plug. See COA #3 for tag. Plug #5 - 620', perf and circulate 195 sx of cement to surface. Plug #6 - 50' of cement at the surface in the casing per COA #4. 8) Submit corrected WBD to correspond with approved 6(N) prior to filing Form 42 Notifications required in COA #1 above.

Bradenhead Testing Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples. If there is a need for sampling, contact COGCC engineering for verification of plugging procedure.
