04/29/2016 | 1) Prior to starting plugging operations, a bradenhead test shall be performed. Call COGCC Engineer with the results of the test before plugging. The Form 17 shall be submitted with Form 6 (s) Subsequent of Abandonment.
2) Provide 48 hour notice of plugging MIRU via electronic Form 42.
3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
4) Stub plug is also shoe plug, this plug must be tagged at 773’ or shallower. Leave at least 100’ cement in the casing for each plug.
