The additional cement referenced shall be placed as indicated and comply with Rule 317.j. The placed cement shall be verified with a CBL and documented with a Form 5 Drilling Completion Report.

1) Prior to starting plugging operations a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test. 2) Provide 48 hour notice of plugging MIRU via electronic Form 42. 3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 4) Operator has indicated plans to run a gyro; please submit gyro survey data with Form 6 Subsequent Report of Abandonment. 5) After pumping squeeze at 1990’, shut down and wait on cement at minimum four hours. Verify gas migration has been eliminated. If evidence of gas migration or pressure remains contact COGCC Engineer for an update to plugging orders. Plug listed as half in-half out surface casing is actually production casing stub plug that will also cover the surface casing shoe. This plug must be tagged at 590’ or 50’ above depth production casing is cut and pulled, whichever is shallower. Leave at least 100’ cement in the wellbore for each plug.
