• Provide 48 hour notice of plugging MIRU via electronic Form 42.
• Production casing stub plug is surface casing shoe plug as well. If it is not circulated to
the surface then tag the plug – must be 715' or shallower.
• Place a 25 sack plug at the surface, all other plugs shall have at least 100' of cement
left in the casing.
• Properly abandon flowlines as per Rule 1103.

Prior to starting plugging operations, a bradenhead test shall be performed. If the initial shut-in bradenhead pressure is greater than 25 psi, notify COGCC Engineer for sampling requirements prior to beginning the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.

06/11/2018 | This approval is good for one year and a new application must be made before March 23, 2019(1 year from prior application). Updates for the planned utilization of the well should be included with the application.
A Form 5A is required to report all plugs currently downhole which isolate the perforated interval.
The Form 7 reporting must reflect the well status dates reported on the Form 4-TA request.
