02/13/2013 | 1) Provide 48-hour notice of MIRU to Jim Precup at (303) 726-3822.
2) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
3) If unable to pull casing, contact COGCC for plugging modifications.
4) Properly abandon flowlines as per Rule 1103.
5) Operator must tag plugs upon verbal request by the COGCC engineer or COGCC field inspector.
