Cement squeezed through perforations at 2500’ up to 2000’ is shown on the well bore diagram with the same color and pattern as existing cement. All cement proposed with this plugging should be shown with the same color and pattern and should look different than previously placed cement. Submit corrected well bore diagram with SROA.

Prior to starting plugging operations a bradenhead test shall be performed. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. 3) If sampling is required contact COGCC engineering for a confirmation of plugging requirements prior to placing any plugs. 4) Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test. 5) Submit Form 42 electronically to COGCC 48 hours prior to MIRU 6) Prior to cutting casing or placing the 1232’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging requirements. 7) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours and tag plug – top of plug must be not deeper than 932’ and provide minimum 10 sx plug at the surface. 8) Leave at least 100’ of cement in the wellbore for each plug. 9) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line the operator must submit a Flowline Report, Form 44.

Planning: • Lighting on location is considered temporary and will be used during recompletion activities. Permanent lighting will not be installed and utilized during normal production operations. Temporary lighting will be directed downward, inward, and shielded towards location to avoid glare on public roads and Building Units within 1,000 feet. Lighting will be turned off when practical, i.e., no operations being conducted.

Traffic Control: • Recomplete operations – The recomplete rig will access the well from the north a county road 20. Dust suppression will be used on county road and temporary access road, if needed. Dust suppression could include a combination of water, magnesium chloride, or calcium chloride. Limited traffic speed could also be used for dust mitigation. • Permanent operations - Production of the location will use the existing access road, located directly off of Weld County Road 20.

Material Handling and Spill Prevention: • Any material not in use that might constitute a fire hazard shall be removed a minimum of twenty-five (25) feet from the wellhead, tanks and separator. Any electrical equipment installations inside the bermed area shall comply with API RP 500 classifications and comply with the current national electrical code as adopted by the State of Colorado.

Noise Mitigation: • A baseline sound survey will be completed at the residence(s) located 490 feet north east of the Haffner 41-23 well. Engineered sound walls around the recomplete rig will be used, as applicable based on the noise survey findings.
