1) Cement from 1650' due to deeper aquifers. Adjust cement volumes accordingly.
2) The additional cement referenced shall be placed as indicated and comply with Rule 317.j. The placed cement shall be verified with a CBL and documented with a Form 5 Drilling Completion Report.
3) Please submit existing gyro survey data with Form 5 Drilling Completion Report. |

12/05/2016 | 1) Prior to starting plugging operations a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test.
2) Provide 48 hour notice of plugging MIRU via electronic Form 42.
3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
4) Leave at least 100’ of cement in the wellbore for each plug. |
