2) Flowlines/pipelines must be abandoned in accordance with 1100 series rules.
3) A copy of the CBL, referenced in the casing history section of this document (number 400492915), for the 2-7/8” casing string supporting the cement top claim of 3680’ must be submitted to COGCC along with the Form 6, Subsequent Report of Abandonment, within 30 days after abandonment of this well.
4) Cement plug from 1040’ to 410’ must extend at minimum from 50 inside casing stub to 50 above surface casing shoe. Plug must be tagged.
