• Form 17 Bradenhead Test Report shall be submitted within 10 days of the test.
• File electronic Form 42 once flowline abandonment is complete.
• Submit CBL run to verify cement coverage with Form 6 SRA.

• Provide 48 hour notice of plugging MIRU via electronic Form 42.
• If there is fluid migration or pressure on the well when shut-in prior to pumping any shoe plug contact COGCC Engineer for revised plugging orders.
• Only if there is no fluid migration or shut-in pressure on the well prior to pumping a stub plug may the stub and surface casing shoe plug be combined. If the combined plug (or shoe plug if not combined) is not circulated to the surface then the plug shall be tagged and must be at least 50’ into the shoe, or 50’ above the cut, whichever is shallower.
• Place a 25 sack plug at the surface, all other plugs shall have at least 100' of cement left in the casing.
• Properly abandon flowlines as per Rule 1103. |

Prior to starting plugging operations, a bradenhead test shall be performed. If the initial shut-in bradenhead pressure is greater than 25 psi or liquid is present at any time during the test, notify COGCC Engineer for sampling requirements prior to beginning the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. |
