06/12/2017 | 1) Prior to starting plugging operations a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test.
2) Provide 48 hour notice of plugging MIRU via electronic Form 42.
3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
4) Submit gyro survey data with Form 6 Subsequent Report of Abandonment.
5) Production casing stub plug is also surface casing shoe plug, this plug must be tagged at 773’ or shallower. Leave at least 100’ of cement in the wellbore for each plug. |

04/13/2018 | This approval is good for one year and a new application must be made before July 25, 2018(1 year from prior application). Updates for the planned utilization of the well should be included with the application. A Form 5A is required to report all plugs currently downhole which isolate the perforated interval. |
