Note change in plugging procedure: 1) Submit Form 42 electronically to COGCC 48 hours prior to MIRU. 2) No CBL on file. Submit CBL to COGCC prior to starting plugging or run CBL to verify the top of primary or squeezed cement is at least 200’ over Niobrara. 3) If unable to pull casing contact COGCC for plugging modifications. 4) For 1310' plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 861’ or shallower. 5) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 6) Please submit existing gyro survey data with Form 6 (s) Subsequent Report of Abandonment. 7) Clarify Dakota abandonment date (1993 stated on 6 (i) well info tab but zones tab says 2005), adjust Dakota production figures accordingly, and submit available contractor and ops reports on Dakota abandonment with Form 6 (s) Subsequent Report of Abandonment.

1) Prior to starting plugging operations a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test. 2) Provide 48 hour notice of plugging MIRU via electronic Form 42. 3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 4) Submit gyro survey data with Form 6 Subsequent Report of Abandonment. 5) No CBL on file with COGCC. Verify cement top with CBL prior to plugging. If TOC is not 4400’ or higher contact COGCC Engineer for update to plugging procedure. Submit CBL with Form 6 Subsequent Report of Abandonment. 6) Production casing stub plug is also surface casing shoe plug. After pumping stub/shoe plug, shut down and wait on cement at minimum 4 hours; verify gas migration has been eliminated. If evidence of gas migration or pressure remains contact COGCC Engineer for an update to plugging orders. Leave at least 100’ cement in the wellbore for each plug.

Prior to starting plugging operations a bradenhead test shall be performed. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples.

File electronic Form 42 once abandonment complete. 3) Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. 4) Please submit gyro survey data with Form 6 Subsequent Report of Abandonment. 5) No CBL on file with COGCC. Verify cement top with CBL prior to plugging. If TOC is not 4400’ or higher contact COGCC Engineer for update to plugging procedure. Submit CBL with Form 6 Subsequent Report of Abandonment. 6) After pumping stub plug, shut down and wait on cement at minimum 8 hours; verify gas migration has been eliminated. If evidence of gas migration or pressure remains contact COGCC Engineer for an update to plugging orders. Leave at least 100’ of cement in the wellbore for each plug.
