10/06/2017 | Prior to starting plugging operations a bradenhead test shall be performed.
1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test. |

10/06/2017 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Properly abandon flowlines as per Rule 1103.
3) Leave at least 100’ of cement in the wellbore for each plug.
4) A copy of all logs run should be submitted to COGCC with Form 6 Subsequent. |

03/20/2018 | This approval is good for one year and a new application must be made before December 6, 2018(1 year from prior application). Updates for the planned utilization of the well should be included with the application.
A Form 5A is required to report all plugs currently downhole which isolate the perforated interval. |
