Prior to starting plugging operations a bradenhead test shall be performed. 1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples.

1) Provide 48 hour notice of plugging MIRU via electronic Form 42. 2) Properly abandon all flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 3) Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. 4) Please submit gyro survey data with Form 6 Subsequent Report of Abandonment. 5) No completed interval report to show the Dakota as TA. Production was reported for the Dakota from September 2012 to April 2017. Production reporting needs to be reviewed and corrected. 6) Must run a CBL to confirm cement from 200-930' and TOC in first string at 4154'. If cement is not visible at those depths on CBL contact COGCC Engineer for update to plugging orders. Submit CBL with Form 6 Subsequent Report of Abandonment. 7) Tag CIBP at 8340’ to confirm there. No records or wireline tickets submitted to COGCC for when this was placed. If CIBP is not tagged where expected contact COGCC Engineer to update plugging orders. 8) Leave at least 100’ of cement in the wellbore for each plug.
