Note change in plugging procedure: 1) Submit Form 42 electronically to COGCC 48 hours prior to MIRU. 2) Add 2 sxs cement onto existing CIBP@5250'. 3) If unable to pull casing contact COGCC for plugging modifications. 4) For 1430' plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 827’ or shallower. 5) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.

1) Prior to starting plugging operations a bradenhead test shall be performed. If the beginning pressure is greater than 25 psi, or if pressure remains at the conclusion of the test, or if any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test. 2) Provide 48 hour notice of plugging MIRU via electronic Form 42. 3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 4) Pressure test exisiting CIBP at 5250' before pumping cement on top. If pressure bleeds off contact COGCC for an update to plugging procedure. 5) Production casing stub plug is also surface casing shoe plug, this plug must be tagged at 827’ or shallower. Leave at least 100’ of cement in the wellbore for each plug.
