Agency | Form: (02 ) 2557967 7/13/2010 | Prior to recomplete, operator must:
1) Verify existing cement with a cement bond log.
2) If it is not present as follows, provide remedial cement 200’ above Niobrara (minimum cement top of 6630’), and 530’ into the surface casing shoe. Verify remedial cement coverage with cement bond log.
3) Recomplete cannot be performed without approved Form 2.

Engineer | Form: (06 ) 401445864 11/17/2017 | Prior to starting plugging operations a Bradenhead test shall be performed.
1) If, before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
3) If sampling is required contact COGCC engineering for an confirmation of plugging requirements prior to placing any plugs.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. The Form 17 shall be submitted within 10 days of the test.

Engineer | Form: (06 ) 401445864 11/17/2017 | Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
|
Engineer | Form: (06 ) 401445864 11/17/2017 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Prior to placing the 900’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging orders.
3) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 271’ or shallower and provide 10 sx plug at the surface. Leave at least 100’ of cement in the casing for each plug.
4) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment is complete.

Engineer | Form: (04 ) 402374025 5/15/2020 | 1. Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. Prior to starting bradenhead mitigation, if a sample has not been collected within the last twelve months collect bradenhead and production gas samples for laboratory analysis. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results shall be provided to the COGCC within three months of collecting the samples.
3. Operator shall implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up.
4. This mitigation plan may be used until 12/31/20 by which time the well must be plugged. By such date a follow-up sundry should be filed to close out mitigation and to document blowdowns conducted prior to plugging. |
Permit | Form: (06 ) 402462484 8/10/2020 | Submit Operator's Monthly Production Reports (Form 7) required for compliance with Rule 309 within 30 days. Submit form 7 reporting consistent with Jsand plug set in September 2010. 10/2010-6/2016, status should be TA for Jsand. |
Engineer | Form: (06 ) 402462484 9/14/2020 | This well must be plugged by 12/31/20. |
Engineer | Form: (06 ) 402462484 9/14/2020 | 1) Provide 48-hour notice of plugging MIRU via electronic Form 42.
2) Operator must tag sand plug at 7190' and dump bail 2 sacks of cement on top of sand. If sand is not tagged contact COGCC engineering prior to continuing plugging operations.
3) After placing the shallowest hydrocarbon isolating plug (6790'), operator must wait a sufficient time to confirm static conditions. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC engineering before continuing operations.
4) The plug at 3000’ needs to be placed and have an 8-hour WOC to assure that all fluid migration has been stopped. If that doesn’t isolate the flow, additional attempts in front of the surface shoe plug will need to be attempted. Other downhole potential squeeze opportunities may need to be looked at before the 725’ plug.
5) Prior to placing the 725’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging orders.
6) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 271’ or shallower and provide at least 10 sx plug at the surface.
7) Leave at least 100’ of cement in the wellbore for each plug.
8) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.
9) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed.

Engineer | Form: (06 ) 402462484 9/14/2020 | Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
|
Engineer | Form: (06 ) 402462484 9/14/2020 | Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations.
1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples.
If there is a need for sampling, contact COGCC engineering for verification of plugging procedure.
