1) Provide 24 hour notice of MIRU to Mike Hickey at 970-302-1024 or e-mail at Mike.Hickey@state.co.us. 2) For 1025’ plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 925’ or shallower. Leave at least 100’ cement in the casing for each plug. 3) Properly abandon flowlines as per Rule 1103.

Note change in plugging procedure: 1) Submit Form 42 electronically to COGCC 48 hours prior to MIRU. 2) Contact COGCC inspector to observe tags of plugs @3220' and 1025'. Both tags must be witnessed by COGCC inspector Tom Peterson. 3) Increase cement used for 3220' plug from 45 sxs to 80 sxs. 4) For 1025' plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 925’ or shallower. If shoe plug not circulated to surface then place 10-40 sx inside casing at surface. Leave at least 100' for each plug. 5) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 6) Please submit existing gyro survey data with Form 6 (s) Subsequent Report of Abandonment. 7) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed with Form 6 (s) Subsequent Report of Abandonment. Diagram should clearly show cement pumped in annulus through perfs below retainer at 6000'.

This is the 6 (s) documenting the replugging of this well, which consisted of drilling out the old plugs and then setting a stub plug and replugging higher up.
