Approval of this Form 4 by COGCC indicates COGCC's acceptance of Operator's documentation that reclamation of the release areas for remediation projects #8518 and #8550 has begun. Approval of this Form 4 does not indicate that COGCC has passed a reclamation inspection for the release areas.

The Operator shall submit notice to COGCC when the sites area ready for a reclamation inspection. Remediation Projects #8518 and #8550 will remain open until both release areas have passed a reclamation inspection.

Bradenhead: Prior to starting plugging operations a bradenhead test shall be performed. The Form 17 shall be submitted within 10 days of the test. 1) If, before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.

Venting during plugging: Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.

Note changes to submitted form. 1) Provide 48 hour notice of MIRU via electronic Form 42. 2) Deep aquifer plug: Tag 1200'-1100' plug. 3) Shoe plug: Tag plug 50’ above surface casing shoe, or circulate cement to surface. 4) Surface plug: Cement from 50' to surface in casing and annulus. 5) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line the operator must submit a Flowline Report, Form 44.
