Note change in plugging procedure: 1) Submit Form 42 electronically to COGCC 48 hours prior to MIRU. 2) Increase cement used to repair casing leak from 25 sxs to 50 sxs. 3) Contact COGCC if switching to a pull casing plugging, as this is a common way to resolve problems with casing leaks. 4) The Sussex formation is productive within one mile of this well. Therefore, cement isolation, which does not currently exist across it, must be provided, by using a perforate and squeeze or equivalent method so that at a minimum there is cement from 200’ below to 200’ above the Sussex formation. 5) For 1075' shoe plug perforate at least 50' below surface casing shoe to ensure annulus coverage. 6) For 1075' plug: pump plug and displace, shut-in, WOC 4 hours and tag plug – must be 910’ or shallower. If shoe plug not circulated to surface then place 10-40 sx inside casing at surface. Leave at least 100' for each plug. 7) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete. 8) Please submit existing gyro survey data with Form 6 (s) Subsequent Report of Abandonment. 9) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
