03/27/2015 | 1) Submit or run cement bond log verifying at least 200’ of cement coverage above the Niobrara and adequate aquifer coverage. Provide remedial cementing if necessary.
2) The additional cement referenced shall be placed as indicated and comply with Rule 317.j. The placed cement shall be verified with a CBL and documented with a Form 5 Drilling Completion Report.
3) Please submit gyro survey data with Form 5 Drilling Completion Report. |

11/10/2016 | 1) Prior to starting plugging operations, a bradenhead test shall be performed. If any pressure remains at the conclusion of the test or any liquids were present contact COGCC Engineer for sampling requirements. The Form 17 shall be submitted within 10 days of the test.
2) Provide 48 hour notice of plugging MIRU via electronic Form 42.
3) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
4) No CBL has been submitted to COGCC that shows 200' of cement above Niobrara. Verify cement top with CBL prior to plugging. If TOC is not 7096’ or higher contact COGCC Engineer for update to plugging procedure. Submit CBL with Form 6 Subsequent Report of Abandonment.
5) Please submit gyro survey data with Form 6 (s) Subsequent Report of Abandonment.
6) Production casing stub plug is also surface casing shoe plug, this plug must be tagged at 645’ or shallower. Leave at least 100’ of cement in the wellbore for each plug. |
