1) Move lower squeeze hole to 1850'. Increase cement volumes accordingly.
2) The additional cement referenced shall be placed as indicated and comply with Rule 317.j. The placed cement shall be verified with a CBL and documented with a Form 5 Drilling Completion Report.
3) Please submit gyro survey data with Form 5 Drilling Completion Report. |

02/17/2016 | Note changes to plugging procedure:
1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Moved CIBP from 7400’ to 7700’, to be within 100’ of the top perf. in the J-sand.
3) For 375’ plug: pump plug and displace. If surface casing plug is not circulated to surface then tag plug – must be 146’ or shallower and provide 10 sx plug at the surface. Leave at least 100’ of cement in the casing for each plug.
4) Properly abandon flowlines as per Rule 1103. File electronic Form 42 once abandonment complete.
