Engineer | Form: (06 ) 402347576 4/7/2020 | Prior to starting plugging operations a Bradenhead test shall be performed if there has not been a reported Bradenhead test within the 60 days immediately preceding the start of plugging operations.
1) If, before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the COGCC within three (3) months of collecting the samples.

Engineer | Form: (06 ) 402347576 4/7/2020 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Prior to placing the 580’ plug: verify that all fluid migration (liquid or gas) has been eliminated. If evidence of fluid migration or pressure remains, contact COGCC Engineer for an update to plugging orders.
3) After isolation has been verified, pump plug and displace. If cement is not circulated to surface, shut-in, WOC 4 hours then tag plug – must be at 162’ or shallower and provide 10 sx plug at the surface.
4) Leave at least 100’ of cement in the wellbore for each plug.
5) Properly abandon flowlines per Rule 1105. File electronic Form 42 once abandonment of on-location flowlines is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line the operator must submit a Flowline Report, Form 44.
6) With the Form 6 SRA operator must provide written documentation which positively affirms each COA has been addressed.
7) After placing the shallowest hydrocarbon isolating plug (1080'), operator must wait a sufficient time to confirm static conditions. If at any time after placing this plug there is evidence of pressure or of fluid migration, contact COGCC engineering before continuing operations.

Engineer | Form: (04 ) 402307202 4/7/2020 | This well must be pluged by 1/1/2021. |
Engineer | Form: (04 ) 402567491 1/11/2021 | 1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. Well to be plugged no later than 12/31/2021. Pressure is to be managed and mitigated until plugging is complete. Summary of well monitoring and mitigation will be submitted on a Form 4 Sundry within 30 days of the well being plugged and abandoned but no later than 12/31/2021. The sundry should include monthly pressure monitoring data, flow rate information, sample analysis interpretation, and bradenhead test description.
3. Shut in bradenhead pressure shall not exceed 50 psig. |
OGLA | Form: (06 ) 402653847 4/9/2021 | 1. Operator will implement measures to control emissions to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public health, welfare, and the environment.
2. Prior to commencing operations, at a minimum, operator will contact the neighborhood HOA and offer information regarding the nature and timing of the P&A operations.
3. Prior to commencing operations, at a minimum, the operator will post signs in conspicuous locations coordinated with the neighborhood HOA or City of Firestone. The signs will indicate plugging and abandonment operations are being conducted, the well name, well location information, and the Operator's contact information. Signs must not create a traffic hazard.
4. Prior to commencing operations, at a minimum, the operator will provide an informational sheet to the Building Units that are adjacent to the parcel on which the well is located. The sheet will include the Operator's contact information, the nature and timing of the P&A operations.
5. This oil and gas location is within a CPW-mapped bald eagle roost or communal roost High Priority Habitat. The P&A activities must not take place between November 15 to March 15 if there is direct line of sight between the active roost to the plugging activity. If plugging activities take place from November 15 to March 15, the Operator must coordinate with CPW in advance and conduct a pre-plugging roost survey to assess roost activity and distance from the roost to the location.
6. This oil and gas location is located in the proximity of CPW-mapped High Priority Habitat for Aquatic Native Species Conservation Water. Operator will review stormwater program and implement stormwater best management practices or erosion control measures as necessary to prevent sediment or impacted stormwater from entering Boulder Creek. |
