1/4/2013 | 1) Provide 48-hour notice of MIRU to Mike Hickey at (970) 302-1024.
2) No CBL on file. Run CBL to verify the top of primary cement is at least 200’ over Niobrara. Submit a copy of the CBL with the Subsequent Report of Abandonment. If the existing cement top is lower than 6799’, additional cement shall be provided to cover this interval and isolate the Niobrara formation.
3) A record in the well file indicated that a CIBP with 2 sacks of cement on top was placed at 7700’ on 11/22/1991. If this plug is still in place, the proposed CIBP at 7730’ is not required.
4) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
5) For the surface casing shoe plug, if not circulated to surface, tag plug, it must be at least 418’ or higher, and a surface plug must be provided.
6) If unable to pull casing, contact COGCC for plugging modifications.
7) Properly abandon flowlines as per Rule 1103.
