06/13/2011 | 1) Provide 24 hour notice of MIRU to Jim Precup at 303-469-1902 or e-mail at James.Precup@state.co.us. 2) The CBL run prior to the plugging must indicate the presence of good cement from the DV tool at 942' into the surface shoe with a minimum TOC of 50'. Both the 990' and the 280'shoe plugs should be tagged. 3) If cement not found as expected then contact COGCC for a modification of plugging procedures. 4)Properly abandon flowlines as per Rule 1103. 5) This well was on the Bradenhead testing list from December 2009 and there is no Bradenhead test form on file for this well - please submit the Bradenhead Test Form.
