| Engineer | Form: (06) 400315725 09/05/2012 | 1) Provide 48 hour notice of MIRU to Mike Hickey at (970) 302-1024.
2) Both the Sussex and Shannon formations are productive within one mile of this well. Therefore, cement isolation, which does not currently exist across them, must be provided for both upon plugging. Also based on offset wells the approximate formation top of the Sussex is 4300’ and the Shannon is 4850’. Therefore, cement may be provided using a perforate and squeeze across each formation (from 50’ above to 50’ below each formation top). Or the volume for the shallower proposed cement squeeze could be increased to cover both formations, as indicated in the plugging procedure modifications above.
3) A CIBP with at least 2 sx of cement is required between the J-Sand and Codell formations.
4) Provide an “As Plugged” wellbore diagram identifying the specific plugging completed.
5) For the surface casing shoe plug, if not circulated to surface, tag plug and it must be at least 107’ or higher.
6) Properly abandon flowlines as per Rule 1103.
7) See plugging changes above.
