This well is located on federal minerals; so, regulatory jurisdiction and higher authority is deferred to the BLM pertaining to downhole procedures. COGCC reporting will remain necessary. A prerequisite for an approval to PA requires the submittal and approval of a COGCC form 6 - Notice of Intent. If CCI would like to PA the well immediately following the conclusion of the proposed workover and while the rig is still on location, the proposed plugging plan as depicted in the attachments to this Sundry Notice is approved in lieu of submitting a form 6 - Notice of Intent; however, a form 6 - Subsequent Report of Abandonment plus a final "As Plugged" well schematic is required plus copies of all job tickets pertaining to wireline and cementing work from service companies. If casing leaks are found during the MIT, then is is required to attempt to cement squeeze the leaks or other means to seal the casing. Check the surface casing via the Braden head access for pressure. If continuous flow is found during blow down, then the PA design must be modified to include the incorporation of perforating and cement squeezing with the proposed cement plugs to elimate the continous flow.
