Operator will assure that the offset well’s Bradenhead is open and monitored during the entire stimulation treatment – a person will monitor for any evidence of fluid, a Bradenhead test will be performed prior to the beginning of stimulation. Any evidence of fluid and or pressure in the listed wells will result in an immediate halt to the stimulation and contact with COGCC area engineer.

01/03/2022 | 1) This approval is good for one year and a new application must be made before 10/11/2022 (1 year from prior application). Updates for the planned utilization of the well should be included with the application.
2) The Form 7 reporting must reflect the well status dates reported on the Form 4-TA request.
3) Submit BH test results on a Form 17 and diagnostic results on a Form 4 Sundry
