| Engineer | Form: (04) 403180081 10/17/2022 | 1) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report
2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
3) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan |
| Engineer | Form: (04) 404034303 02/05/2025 | No later than 7/24/2025, conduct a bradenhead test, submit a Form 17, and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the new proposed mitigation plan due to continuous flow at the end of the test, sample analysis interpretation, bradenhead test description, and the flow rate information and pressure data. |
| Engineer | Form: (04) 404034303 02/05/2025 | 1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up.
3. If a sample has not been collected from surface casing within the last twelve months collect bradenhead gas samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the ECMC within three months of collecting the samples.

| Engineer | Form: (04) 404439726 03/18/2026 | 1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. Prior to starting bradenhead abatement, if a sample has not been collected within the last twenty four months collect bradenhead and production gas samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the ECMC within three months of collecting the samples.
If a sample has not been collected from production gas, collect production gas samples for laboratory analysis. Operator may submit a production gas from an adjacent well if on a multi-well pad. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
3. Bradenhead gas is not to be vented to atmosphere; any gas from the Bradenhead will be routed to the specified abatement system. Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. The abatement program may be used for six consecutive months.
4. Operator will submit a Form 42 (“OTHER”) stating that “bradenhead abatement program has started.”
5. At the conclusion of the six months (from installation of pressure mitigation system), conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans, flow rate information, pressure data and a discussion of the sample analysis.
6. At least one check valve is required for annular spaces that are tied to sales line or separator. Maintain equipment for pressure regulation and check valves in good working order.
7. At least one valve is required to monitor pressure and sample flow from the bradenhead.
8. Within 30 days of completing the work, submit a Form 4 Subsequent Report including a schematic of the bradenhead tie in
