2) For Wells that have sustained casing pressure above the action threshold as determined through diagnostic testing and evaluation, the Operator will provide a PMP with the Form 4, including a description of the Operator’s findings from the previous year, number of times the well was blown down, a current WBD and future plans for more frequent monitoring or testing, pressure mitigation, or remedial well work by 3/10/2022
3) Submit the gas sample analysis from previous Form 17 bradenhead test on a Form 43
4) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.

2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
