1) This sundry to conduct bradenhead diagnostic evaluations is valid until 12/02/2021 2) For Wells that have sustained casing pressure above the action threshold as determined through diagnostic testing and evaluation, the Operator will provide a PMP with the Form 4, including a description of the Operator’s findings from the previous year, number of times the well was blown down, a current WBD and future plans for more frequent monitoring or testing, pressure mitigation, or remedial well work by 12/02/2021 3) Submit the gas sample analysis from previous Form 17 bradenhead test on a Form 43 4) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.

1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Bradenhead gas is not to be vented to the atmosphere; any gas from the Bradenhead will be routed to the specified abatement system. Shut in bradenhead pressure shall not exceed threshold pressure. Operator will implement measures to get an estimate of the gas flow rate and/or volume from the bradenhead. 3. No later than 10/20/26, conduct a bradenhead test, submit a Form 17, and submit a Form 4 Sundry that summarizes current well condition. The well should be shut in for seven days to monitor and collect data to characterize build up pressures prior to conducting the bradenhead test. The sundry should include details of the future plans, sample analysis interpretation, bradenhead test description, and the flow rate information and pressure data.
