11/15/2022 | 1) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report
2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
7) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan -Include diagnostic information on causal factors for intermediate casing pressure.
5) Complete installation of BH monitoring system by 3/31/2023
4) Perform diagnostics on BH and Intermediate pressures being the same and report results on Sundry required by COA#3 |

01/28/2026 | 1) For applicable operations, report work completed within 30 days on a Form 4 BHP Well noting final pressure test results and include any operational or job summaries. If the wellbore geometry changes or there is not an updated WBD in the well file submit an updated WBD. The sundry should include details of the future plans, bradenhead test description, and pressure data.
2) Complete a bradenhead test within 30 days of completing the repair and submit results on a Form 17
3) Operator shall notify ECMC Engineering staff of changes and/or additional repair methods to the approved repair procedure. |
