2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method.
3) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan -Include diagnostic information on causal factors for intermediate and surface casing pressure.
4) Complete installation of BH monitoring system by 2/31/2023

| Engineer | Form: (04) 404449695 05/07/2026 | 1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare.
2. Bradenhead gas is not to be vented to the atmosphere. Shut in bradenhead pressure shall not exceed threshold pressure. Operator will implement measures to get an estimate of the gas flow rate and/or volume from the bradenhead.
3. No later than 6/18/2026, (12 months after the most recent BH test) conduct a bradenhead test, submit a Form 17 within ten days of the test.
Within 60 days of conducting the Bradenhead test, submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans, sample analysis interpretation, bradenhead test description, and the flow rate information and SCADA pressure data.
