2) This well has federal minerals. Operator shall notify COGCC engineering staff of any procedure changes required by the BLM or unexpected conditions in the field as soon as feasible.
3) Submit revised formation tops, CBL and changes to wellbore on Form 5 if cement is pumped. Form 4 if cement is not pumped.
