5/4/2021 | 1. Flaring is approved until January 15, 2022 or the CDPHE allowed quantity of gas is exceeded. Flaring of natural gas shall comply with COGCC Rule 903.
2. The unnecessary or excessive venting or flaring of produced natural gas is prohibited.
3. Operator is to provide prior notification to the local emergency dispatch or the local governmental designee of any flaring or venting event.
4. An Operator Monthly Report of Operations, Form 7, requires the reporting of flared, vented or used on the lease gas volumes. The reported volume includes flowback and production gas.
5. The operator is required to obtain and maintain any required air permits from CDPHE.
6. If flaring is requested after Jan 15, 2022, submit a gas capture plan via a Form 4. The Operator may not Flare or Vent pursuant to Rule 903.d.(3) unless and until the Director approves the Form 4.

11/14/2023 | 1) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report or other Director approved submittal method.
2) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges.
5) At least one valve is required to monitor pressure and sample flow from the bradenhead.
6) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan
7) Submit required forms and documentation to register new flowlines
8) Submit a Form 4 for any venting during maintenance activities if it exceeds 8 hours
9) Submit a new sundry with an updated Gas Capture Plan if there are significant changes to wellbore conditions or the gas take away system
10) Complete a Form 17 Bradenhead Test upon completion of gas capture plan and bradenhead plan work
