1) This sundry to conduct bradenhead diagnostic evaluations is valid until 4/21/2022 (6 months from bradenhead test date) 2) For Wells that have sustained casing pressure above the action threshold, flows liquids or continuous gas from the bradenhead annulus as determined through diagnostic testing and evaluation, the Operator will provide a PMP with the Form 4, including a description of the Operator’s findings from the previous 6 months, number of times the well was blown down, a current WBD and future plans for more frequent monitoring or testing, pressure mitigation, or remedial well work by 4/21/2022 3) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges. 4) Complete a Form 17 Bradenhead test upon completion of diagnostic work

1) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report 2) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method. 3) Comply with any CDPHE, Air Pollution Control Division rules or requirements for all atmospheric discharges. 4) Any liquids blown down are to be collected, stored, handled, and treated or disposed as E&P waste per COGCC’s 900 series rules. 5) At least one check valve is required for annular spaces that are tied to sales line or separator. Maintain equipment for pressure regulation and check valves in good working order. 6) At least one valve is required to monitor pressure and sample flow from the bradenhead. 7) Within 30 days of completing the work, submit a Form 4 Subsequent Report - Bradenhead Plan

1) This well is designated inactive and must be properly returned to production, plugged and abandoned, have an approved Form 5B or be submitted on a Form 6A (to be designated as Out of Service) no later than no later than November 1, 2022. 2) The Form 7 reporting must reflect the well status dates reported on the Form 4-TA request.

1. Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Bradenhead gas is not to be vented to the atmosphere; any gas from the Bradenhead will routed to to specified abatement system. Shut in bradenhead pressure shall not exceed threshold pressure. Operator will implement measures to get an estimate of the gas flow rate and/or volume from the bradenhead. 3. No later than 3/16/2025, conduct a bradenhead test, submit a Form 17, and submit a Form 4 Sundry that summarizes current well condition. The well should be shut in for seven days to monitor and collect data to characterize build up pressures prior to conducting the bradenhead test. The sundry should include details of the future plans, sample analysis interpretation, bradenhead test description, and the flow rate information and pressure data. 4. If a sample has not been collected from surface casing within the last twelve months collect bradenhead gas samples for laboratory analysis. Sampling will comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Copies of all final laboratory analytical results will be provided to the ECMC within three months of collecting the samples.
