5) Operator shall notify ECMC Engineering staff of changes and/or additional repair methods to the approved repair procedure.
Contact ECMC engineering if a casing leak is identified and cement or a liner will be used to repair well
6) Complete a Form 21 MIT to verify repairs for injection wells
