1. Injection is not authorized until approval of Subsequent Forms 31 and 33. 2. Retrieve water sample(s) from injection zone(s) before stimulating formation. Samples must be analyzed for Total Dissolved Solids at a minimum. 3. PRIOR TO PERFORMING OPERATIONS: Approval of Form 4 is required for acid and fracturing jobs. (New as of 4/13/2016). 4. PRIOR TO PERFORMING OPERATIONS: Operator is required to contact COGCC to discuss Step Rate Test or Injectivity Test criteria for Maximum Surface Injection Pressure determination. Prior approval of Form 4 is required for step rate and injectivity tests. 5. For ALL NEW DRILL UNDERGROUND INJECTION WELLS an open-hole Resistivity/Gamma Ray log IS REQUIRED from Surface Casing shoe to TD. A PDF, TIFF, or PDS visual image and a LAS or DILS file version of each log is required. 6. For all new and converted Underground Injection Control wells a Cement Bond Log (CBL) is required on the cased portions of the hole from the bottom of the casing to the top of the next shallower casing string for all casing strings other than the Surface Casing. Only a PDF, TIFF, or PDS visual image is required. 7. Operator must provide all tops of formations encountered from surface to TD on the Form 5 when submitted.

Logging open hole resistivity with gamma ray will be run on this well to describe the stratigraphy of the wellbore and to adequately verify the setting depth of the surface casing and aquifer coverage. A CBL will be run on all production casing or, in the case of a production liner, the intermediate casing, when these casing strings are run.

Rule 604.c(2)I Rule 604.c.I: Upon initial rig-up and at least once every thirty (30) days during drilling operations thereafter, pressuretesting of the casing string and each component of the blowout prevention equipment including flange connections shall be performed to seventy (70%) of working pressure or seventy (70%) of the internal yield of casing, whichever is less. Pressure testing shall be conducted and the documentation results shall be retained by the operator for inspection by the Director for a period of one (1) year. Activation of the pipe rams for function testing shall be conducted on a daily basis when practicable.

Rule 1003.d(1) Drilling pit closure on crop land and within 100 year floodplain. Drilling pit reclamation, including the disposal of drilling fluids and cuttings, shall be performed in a manner so as to not result in the formation of an impermeable barrier. Any cuttings removed from the pit for drying shall be returned to the pit prior to backfilling, and no more than de minimis amounts may be incorporated into the surface materials. After the drilling pit is sufficiently dry, the pit shall be backfilled. The backfilling of the drilling pit shall be done to return the soils to their original relative positions. Closing of the drilling pit shall occur no later than three (3) months after drilling and completion activities conclude.
