| Engineer | Form: (02) 400391335 03/18/2013 | (1) COMPLIANCE WITH THE MOST CURRENT REVISION OF THE NORTHWEST COLORADO NOTIFICATION POLICY IS REQUIRED.
(2) PROVIDE CEMENT COVERAGE FROM THE INTERMEDIATE CASING SHOE TO A MINIMUM OF 200’ ABOVE TOP OF OIL or GAS (OR OTHER SHALLOWEST-KNOWN PRODUCING HORIZON). VERIFY CEMENT WITH A CBL. |
| Permit | Form: (05) 400404607 06/03/2013 | As drilled GPS must be submitted by end of year. |
| Engineer | Form: (06) 401160564 01/22/2017 | 1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Properly abandon flowlines per Rule 1103. File electronic Form 42 when flowline abandonment is complete.
3) Submit "As-Drilled" GPS data per Rule 311.d.
4) Check the 10+3/4" x 7+5/8" (bradenhead) annulus pressure, and report pressure on Form 6 Subsequent Report of Abandonment; notify COGCC Engineering staff if bradenhead gas or liquid flow is observed prior to perforating at 1832'.
5) Pressure test 7+5/8" First String prior to perforating at 1832', as described on operator's procedure shown on the attached Wellbore Diagram #401160718, and notify COGCC engineering staff if casing does not hold pressure. Plugging changes may be required.
6) Based on COGCC review of the 7+5/8" casing Cement Bond Log (CBL) #400426818, circulation is not expected after perforating at 1832'. Pump balanced plug in casing from planned CIBP at 1882' to 1732' if rate is not established for squeeze into perforations at 1832'. |
