1) At the conclusion of the 6 months from BH test date (3/23/23), submit a Form 4 BHP Sundry that summarizes current well condition. The sundry should include details of the future plans, flow information, number of times well was blown down, pressure data and a discussion of the abatement history for the previous 6 months. 2) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing, and Reporting and Rule 420. Form 17, Bradenhead Test Report 3) At least once a year shut in bradenhead for 7 days or until the pressure reaches the bradenhead threshold and perform a bradenhead test. Report results on a Form 17, as specified in Rule 420 or other Director approved submittal method

Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. By 8/8/2004 , conduct a new bradenhead test and submit the Form 17 within ten days of the test and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the future plans, sample analysis interpretation, and the flow rate information and pressure data. 3. Operator will implement measures to get an estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up.

Operator will implement measures to get an initial estimate of the gas flow rate and/or volume from the bradenhead. During the shut-in period record pressure data to adequately characterize the build-up. 3. No later than 7/2/2026, conduct a bradenhead test, submit a Form 17, and submit a Form 4 Sundry that summarizes current well condition. The sundry should include details of the new proposed mitigation plan due to continuous flow at the end of the test, bradenhead test description, and the flow rate information and pressure data.
