2) Provide cement coverage of production casing from TD to 50' above surface casing shoe. Verify coverage with cement bond log.
3) Immediately cease operations and notify the COGCC in the event of any operational problem that could potentially impact water wells in the area.

10/20/2023 | 1) Operator shall comply with COGCC Rule 419. Bradenhead Monitoring, Testing,
and Reporting and Rule 420. Form 17, Bradenhead Test Report
2) At least once a year shut in bradenhead for 7 days or until the pressure reaches
the bradenhead threshold and perform a bradenhead test. Report results on a Form 17,
as specified in Rule 420 or other Director approved submittal method.
3) Complete subsequent Form 21 MITs in accordance with the timelines set forth in
Rule 417. If well is not returned to production, an MIT is due by 7/19/2028
4) Submit an updated WBD on a Form 4 Sundry.
