Perform a bradenhead test and submit a Form 17 prior to submittal of the next year’s request for continued TA status. A new application must be made before July 6, 2020. Updates for the planned utilization of the well should be included with the application. A Form 5A, Completed Interval Report, is required to report all plugs currently downhole which isolate any perforated interval. The Form 7, Operators Report of Monthly Operations, must correctly reflect the well status and dates reported on the Form 4, Sundry Notice-Request for Continued Temporarily Abandoned Status.

1) Provide 48 hour notice of MIRU via electronic Form 42. 2) Shoe plug: Tag plug 50' above surface casing shoe 3) Surface plug: Cement from 50' to surface in casing. Bradenhead: 1) If there has not been a reported Bradenhead test within 60 days of plugging this well, prior to starting plugging operations a Bradenhead test shall be performed. 2) If before opening the Bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required. 3) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling. Venting during plugging: Operator shall implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare. Flowlines: Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment is complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line, the operator must submit a Flowline Report, Form 44.
