If the well was not sampled as part of a prior test it should be sampled if pressure is 25# or greater. If there is a need for sampling - contact COGCC engineering for verification of plugging procedures (Pressure greater than 25# or any liquids flowed to surface).
1) If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
2) If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions, Appendix A: Liquid and Gas Sampling.
The Form 17 shall be submitted within 10 days of the test.

| Engineer | Form: (06) 401518501 03/20/2018 | If there is an intention to repair this well and retain the wellbore for future uphole development, the portion of the well under the identified hole at 4190' and above bridge plug/cement at 5965' shall be tested to 350# and demonstrate mechanical integrity prior to placing the balanced plug at 4240'. Test should be documented. The well can be left after placing the 4240' plug without additional plugs, if there is demonstrated mechanical integrity.
If the wellbore is to be retained - removed requirement for plug at 5275'. Wellbore must demonstrate mechanical integrity above plug placed at 4240' to a minimum of 350#. Test should be documented.
If the well is to be retained to be developed in an uphole reservoir, there should be a Form 4, Sundry Notice filed on this well removing its status as a UIC well . The Form 4 should also detail all plugs placed in the well and provide the contractor and wireline summaries for those plugs. A wellbore diagram describing the wellbore configuration after the work should be included.
If the well is to retained/maintained as a temporarily abandoned well, the well will need to have a mechanical integrity test that is noticed appropriately (electronic Form 42) to give the field inspection unit an opportunity to witness the MIT. If not witnessed, test must be charted. A Form 21, Mechanical Integrity Test should be submitted once the MIT is completed. |
