2)Submit a Form 42(CMT) for all strings on this well. Per Rule 405.g, Operators will provide the Director written notice 24 hours in advance of running and cementing casing on this well. Such notice will be provided on a Form 42, Field Operations Notice– Notice to Run and Cement Casing.
3)If production casing is set provide cement coverage from TD (production casing shoe) to 200’ above the surface casing shoe or the base of the Coalmont, whichever is shallower. Run and submit CBL to verify cemented intervals.
4)Include all identifiable formation tops from the surface to TD on the Form 5.
