2)If a delayed completion, a second test is required between 6-9 months after rig release.
3)A post-production test within 60 days after first sales.

2)If production casing is set provide cement coverage from TD(production casing shoe) to 200’ above the surface casing shoe or the base of the Coalmont, whichever is shallower. Run and submit CBL to verify cemented intervals.
3)Include all identifiable formation tops from the top of the Coalmont to TD on the Form 5.
