2) Submit annual updates (requests to flare) to COGCC on Form 4’s (Sundry Notices), which discuss whether or not any new gathering systems or increased development of the field have changed pipeline economics such that installation of a new pipeline would be an economically viable option in lieu of continued flaring. The annual update shall include gas analysis for hydrogen sulfide.
3) Comply with any Colorado Department of Public Health and Environment, Air Pollution Control Division rules or requirements for all atmospheric discharges.
4) An enclosed flare shall be used, unless an open flare is specifically allowed by CDPHE’s Regulation 7. |

1) Provide 48 hour notice of plugging MIRU via electronic Form 42.
2) Plugs and squeezes will be placed as stated in the plugging procedure of the approved NOI unless revised by COA or prior approval from COGCC is obtained
3) Check bradenhead pressure prior to MIRU. Perform a bradenhead test if pressure is greater than 50 psi, submit results electronically on a Form 17, and contact COGCC area engineer.
4) Operator shall implement measures to control unnecessary and excessive venting, to protect the health and safety of the public, and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard to public welfare.
5) Properly abandon flowlines as per Rule 1105. File electronic Form 42 once abandonment complete. Within 30 days of an operator completing abandonment requirements for an off-location flowline or crude oil transfer line the operator shall submit a Flowline Report, Form 44.
6) Place a 50' plug (minimum) at the surface, both inside the inner most casing and in all annular spaces. All other cement plugs, without mechanical isolation, shall have at least 100' of cement left in the casing.
7) Isolate the liner with a packer and attempt to establish injection into reported holes at 6984’ after setting CIBP at 7350’ and before perforating liner at 7300’. If rate/pressure is satisfactory then these holes may be used to squeeze as outlined in the procedure.
8) COGCC does not have a CBL on file that verifies the cement coverage for the 7” string. Operator will verify cement coverage of the 7” with a CBL and submit with the Form 6 SRA. If the cement coverage is not found to be as represented on this approved NOI then the operator will contact the COGCC Engineer for revised plugging orders prior to proceeding with plugging operations.
9) Test the 7” casing to at least 300 psi after isolating top of liner. Contact COGCC engineer if pressure test fails for revised plugging orders prior to proceeding with plugging operations.
10) Insure that there is no pressure or fluid migration on the casing or bradenhead prior to perforating at 465’. Contact COGCC engineer if not the case for revised plugging orders prior to proceeding with plugging operations.
