| Permit | Form: (02) 401410538 02/01/2018 | Operator shall comply with Notice to Operators: Interim Reclamation Procedures for Delayed Operations (dated January 5, 2017). |
| Engineer | Form: (02) 401410538 03/02/2018 | 1) Operator shall comply with the most current revision of the Northwest Notification Policy.
2) Operator shall comply with the most current revision of the Garfield County Rulison Field Notice to Operators, with the following exception: All field notice requirements specified in that Notice to Operators are superseded by the requirements of the most current revision of the Northwest Colorado Notification Policy (see Condition of Approval #1).
3) Operator shall provide cement coverage from the production casing shoe (4+1/2" First String) to a minimum of 200' above the Wasatch G formation. Adjust cement quantities as needed. Verify production casing cement coverage with a cement bond log.
4) The Operator shall monitor the bradenhead pressure of the proposed well and all offset wells under Operator’s control which penetrate the stimulated formation and have a treated interval separation of 300 feet or less. Monitoring shall occur from 24 hours prior to stimulation and shall continue until 24 hours after stimulation is complete. Recording shall be at a frequency of at least once per 24 hours with the capability of recording the maximum pressure observed during each 24 hour period. Operator shall notify COGCC Engineering staff if bradenhead pressures increase by more than 200 psig.

| Engineer | Form: (04) 402651655 10/26/2021 | 1) The COA requiring cement coverage 200' above the top of the Mesa Verde (Wasatch
G or Ohio Creek if present) is waived for this well. Operator is to monitor the bradenhead and notify COGCC engineering staff if the pressure exceeds 0.3 times the surface casing setting depth (TVD) in psi. |
| OGLA | Form: (06) 403741500 04/12/2024 | This oil and gas location is located within a CPW-mapped 500 foot buffer of the Ordinary High Water Mark of the West Fork Parachute Creek for protection of Aquatic Native Species Conservation Water High Priority Habitat. At a minimum, Operator will review the stormwater program and implement stormwater BMPs and erosion control measures as needed to prevent fine-grained sediment and impacted stormwater runoff from entering surface water. Confirmed via email correspondence with CPW 4/24/2024. |
| OGLA | Form: (06) 403741500 04/12/2024 | Due to proximity to a wetland, surface water and expected shallow groundwater, operator will use secondary containment for all tanks and other liquid containers. |
| Engineer | Form: (06) 403741500 05/08/2024 | 1)Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations and electronic Form 42 Notice of Plugging Operations 48 hours prior to mobilizing for plugging operations. These are two separate notifications, required by Rules 405.e and 405.l.
2)The approved Form 6, Notice of Intent will be at the location during all phases of plugging operations.
3)Operator shall implement measures to control venting and to ensure that vapors and odors from well plugging operations do not constitute a nuisance or hazard.
4)Prior to placing surface shoe plug verify that all fluid (liquid and gas) migration has been eliminated. If evidence of fluid migration or pressure remains, contact ECMC Engineer for an update to plugging orders.
5)If applicable properly abandon flowlines as per Rule 1105. Pursuant to Rule 911.a. Closure of Oil and Gas Facilities, Operator will submit Site Investigation and Remediation Work plans via Form 27 for ECMC prior approval before cutting and capping the plugged well, conducting flowline abandonment, and removing production equipment. Pursuant to Rule 1105.f. Abandonment Verification, within 90 days of an operator completing abandonment requirements for a flowline or crude oil transfer line, an operator must submit a Field Operations Notice, Form 42-Abandonment of Flowlines for on-location flowlines, and a Flowline Report, Form 44, for off-location flowlines or crude oil transfer lines.
6)Prior to starting plugging operations a bradenhead test shall be performed if there has not been a reported bradenhead test within the 60 days immediately preceding the start of plugging operations.
If, before opening the bradenhead valve, the beginning pressure is greater than 25 psi, sampling is required.
If pressure remains at the conclusion of the test, or if any liquids were present during the test, sampling is required.
The Form 17 shall be submitted within 10 days of the test. Sampling shall comply with Operator Guidance - Bradenhead Testing and Reporting Instructions. If samples are collected, copies of all final laboratory analytical results shall be provided to the ECMC within three (3) months of collecting the samples.
7)With the Form 6 SRA operator must provide written documentation, which positively affirms each COA listed above has been addressed.
8)This well has federal minerals. Operator shall notify ECMC engineering staff of any plugging changes required by the BLM or unexpected conditions in the field as soon as feasible. Primary contact is Aaron Katz at 970-765-6300. Alternate ECMC contact is Byron Gale at 970-319-4194.
