(1) Operator shall comply with the most current revision of the Northwest Notification Policy. See attached notice. (2) Operator shall comply with the most current revision of the Garfield County Rulison Field Notice to Operators, with the following exception: All field notice requirements specified in that Notice to Operators are superseded by the requirements of the most current revision of the Northwest Colorado Notification Policy (see Condition of Approval #1). See attached notice. (3) Operator shall provide cement coverage from the production casing shoe (4+1/2" first string) to a minimum of 200' above all Mesa Verde Group (and Ohio Creek Formation, if present) oil, gas, and water-bearing sandstone and coalbed formations. Verify production casing cement coverage with a cement bond log.

1) For applicable operations, report work completed within 30 days on a Form 4 Sundry REPORT OF WORK DONE - Repair Well noting final pressure test results and include any operational or job summaries. If the wellbore geometry changes or there is not an updated WBD in the well file submit an updated WBD. 2) For applicable operations, submit a Form 5 or 5A (updated packer and tubing depths) within 30 days of the work with job related documents including a minimum of one of the following attachments: operations summary, cement job summary and/or wireline summary. Additionally, include CBL’s and any additional logs run. 3) Complete a bradenhead test within 30 days of completing the repair and submit results on a Form 17 4) Provide electronic Form 42 Notice of MIRU 2 business days ahead of operations 5) Operator shall notify ECMC Engineering staff of changes and/or additional repair methods to the approved repair procedure.

Operator will implement measures to control venting, to protect health and safety, and to ensure that vapors and odors from well operations do not constitute a nuisance or hazard to public welfare. 2. Well to be plugged within 6 months of identifying integrity issue - 9/6/2025. Pressure is to be managed and mitigated until plugging is complete. Summary of well monitoring and mitigation will be submitted on a Form 4 Sundry within 30 days of the well being cut and capped. The sundry should include monthly pressure monitoring data, flow rate information, sample analysis interpretation, and bradenhead test description. 3. Shut in bradenhead pressure shall not exceed threshold pressure.

Open hole resistivity log with gamma ray will be run on one well in the “first occupation” of the pad to describe the stratigraphy of the vertical section of the wellbore and to adequately verify the setting depth of the surface casing and aquifer coverage. The Drilling Completion Report - Form 5 for every well on the pad will identify which well was logged.
