1)The COA requiring cement coverage 200' above the top of the Mesa Verde (Wasatch G or Ohio Creek if present) is waived for this well. Operator is to monitor the bradenhead and notify COGCC engineering staff if the pressure exceeds 0.25 times the surface casing setting depth (TVD) in psi. 2)Operator is to perform a bradenhead test and submit the results on a Form 17 within one month of this form approval.

Remaining wells on the pad will be logged with either cased hole pulsed neutron or triple-combo open hole. Every well will also have a CBL log from well TD up through well surface. Form 5 Completion Reports will identify wells on the pad with triple-combo open hole logs.

To meet the Rule 317.p open-hole logging requirements on the GR 23-11V multi well pad, TEP will use logs from the existing Federal GR 23-11V well (API # 05-045-06811). All wells on the pad will have a cement bond log (CBL) with gamma-ray run on production casing (or on intermediate casing if production liner is run) into the surface casing. The Form 5, Completion Report, for each well on the pad will list all logs run and have those logs attached. The Form 5 for a well without open-hole logs will state “No open-hole logs were run” and will clearly identify the type of log and the well (by API#) in which open-hole logs were run.
